Cephalosporins, like penicillin, may induce the formation of antibodies which may cause a positive direct antiglobulin test (Direct Coombs) and occasionally hemolytic anemia in the patient receiving the drug. The laboratory can test for antibody by mixing patient serum with cephalothin coated red blood cells. Agglutination at any phase of the test indicates the presence of a cephalothin antibody. If a cephalothin antibody is present, another class of antibiotic should replace cephalothin since red blood cell destruction may occur.
Specimen requirement is 5 mL of whole blood collected in a lavender top tube.
