Autoantibodies to the carboxy end of type IV procollagen molecules in the glomerular basement membrane cause glomerulonephritis, Goodpasture's syndrome, and less commonly, idiopathic pulmonary hemosiderosis. This test is useful in the evaluation of patients with rapidly progressive glomerulonephritis with or without pulmonary hemorrhage. Elevated levels, often >250 Eu/mL, are detected in patients with glomerulonephritis with pulmonary hemorrhage (Goodpasture's syndrome) glomerulonephritis without pulmonary involvement, and in idiopathic pulmonary hemosiderosis. Weakly positive results (5 - 30 Eu/mL) may occur in some patients without anti-glomerular basement membrane antibody mediated diseases.
